Fresh Food Pharmacy
Grace Health teamed up with South Michigan Food Bank and the Food Council of Michigan to offer Grace Health’s Fresh Food Pharmacy to support our patients who are diabetic, pre-diabetic, have high blood pressure and/or heart disease. The Fresh Food Pharmacy is a 6-month program that helps patients take control of their health, one meal at a time. During the program, patients meet with a dietician or a Healthy Lifestyles coach once a month to learn about healthy habits and gain support on their journey to better health.
