Grace Health Recognized for Quality Care

Grace Health was recently recognized by the American Heart Association, Priority Health, and Blue Cross Blue Shield for improving care in the community.

The American Heart Association awarded Grace Health the Gold 2024 Check. Change. Control. Cholesterol Achievement Award for having greater than 70% appropriate statin therapy use throughout the year for Grace Health’s at-risk patient population.  The American Heart Association commended Grace Health on its commitment to lowing cholesterol and improving care management within its patient population.  Grace Health was awarded the Silver 2024 Target: BP award by the American Heart Association and American Medical Association.  This award was given for Grace Health’s commitment to achieving better blood pressure control among patients.  The ultimate goal is to reduce the number of people who have heart attacks and strokes.

Grace Health was named the recipient of the 2023 Quality Award by Priority Health, which recognizes top physician offices that improve health care outcomes for patients across Michigan.  The annual awards are given to physicians and physician offices that receive the highest overall scores through the Priority Health Primary Care Provider Incentive Program. Offices must meet or exceed the national 90th percentile across 13 performance outcomes in preventive care and chronic disease measures, including cancer screening, immunizations and chronic condition management. “We’re thrilled to work with such amazing physicians who truly care about making Michigan a healthier place,” said Praveen Thadani, president/CEO of Priority Health. “This recognition is our way of celebrating their exceptional care and acknowledging their dedication and expertise.”

Grace Health was once again recognized as a Patient-Centered Medical Home and Collaborative Care practice by Blue Cross Blue Shield of Michigan for Grace Health’s commitment to delivering patient-centered primary care, engaging patients in health care, and improving medical outcomes for Michigan residents.  Grace Health is also recognized as a Patient-Centered Medical Home with Distinction in Behavioral Health Integration though the NCQA (National Committee for Quality Assurance) for its integration of behavioral health in its medical practice as well as in its school sites.
